How to Upload Your Proof of Vaccination
Use the steps below to upload your proof of vaccination. Before uploading your proof of vaccination, you will need to log into your Clearing User Account. If you need help creating one or logging into an existing account, please visit: Clearing User Account or give us a call 920-854-4088!
If uploading using your desktop, you will need a photo or scan of your vaccination card stored on your computer. If uploading using your phone, you can either use an existing photo or take a new photo of your vaccination card. Proof of Vaccination Upload Instructions:
Click on the 'Account' tab, and click on 'Log In' link.
Log into your Clearing User Account. Enter your username/email, your password and click the 'Log In' button.
Click the 'Menu' icon in the main menu bar.
Click the 'Document Center' link.
Click the 'Upload Proof of Vaccination' link.
Click the button next to your name. Click the 'Browse' button to locate your Proof of Vaccination file. (You can select a file on your desktop or choose to use an existing photo or take a new photo from your phone.) Click the 'Upload Document' button.
Once uploaded, a green banner noting 'The action was completed successfully.' will appear. Thank you for submitting your proof of vaccination!
If you would like to review your uploaded documents, you can always find them in the Document Center by clicking on the 'Upload Document' button.
